Dear Parents,

The doctors and health care professionals of Delaware Pediatrics would like to thank you for trusting us with the privilege of caring for your family.  It saddens us to inform you that for a variety of reasons, some beyond our control, we will be closing our office at 4845 Transit Road, Depew effective June 30, 2017.  All of the doctors and health care professionals that you have become accustomed to seeing at the Transit Road office will be available to you and your children at our office on Delaware Avenue.  We sincerely regret leaving the Transit Road location and apologize for any inconvenience that this news may cause you.

We hope that you will decide to stay with our practice and would be happy to welcome your family to our North Buffalo, 2550 Delaware Avenue site where you can continue to see the health care provider you have been seeing at our Transit Road office.  A letter to all our Transit Road patients will be going out in today’s mail. 

In the event that you are unable to travel to our Delaware Avenue location, we will ensure that your child’s medical records are transferred to the Pediatrician of your choice. Please click on the following link to print a copy of our  Release of Medical Records Form.  The form must be signed by you and returned to our office by June 9, 2017, in order to allow for sufficient time to have your child’s medical records transferred prior to our closing date.

If your child is on any form of daily medication and you have decided to transfer your child’s care to another pediatric practice, please notify us as soon as possible at, (716) 884-0230  so that there will not be any gaps or delays in renewing your child’s prescription.

Again, it has been our pleasure and privilege to care for your family and we regret any inconvenience that may come with this news.
